Twelve certificates, one to a page
One contents page, then twelve full-page certificates. Each carries a title, a sentence stating what the recipient did, a supporting NKJV verse, and blank lines for the name, the date, and a signature. The signature line is labeled for whoever would actually sign it — Baptized by, Witnessed by, Pastor, Teacher, or Director.
Who it suits

In practice
Print for the moment, not for the file
White or ivory cardstock gives a keepsake finish. A church can print a stack of baptism certificates once and keep them ready in the office. A homeschool or Sunday school prints the memory and reading awards at the close of a term. Fill the lines in by hand and sign before you present it.
Honest limits
Nothing is typed
There are no editable fields and no way to merge a class list, because the name, date, and signature are meant to be written by the person presenting the certificate. The wording on each page is fixed. If your church words baptism or dedication differently, this file will not match it.
